Diabetes Self-Management Education and Support

This is a self-paced, video-based Diabetes Education course and has been personalized for you. There are many resources for Affordable Food Programs and Affordable Medication Programs within this course. You may also use the comments section to speak with a health professional.

This course provides information on Insulin/Medication Assistance Programs from Eli Lilly, Nova Nordisk, and Sanofi (at the end of Lesson 5)

This course is comprised of short videos of diabetes experts and people like you living with diabetes, with a review and a quiz for every topic. Each lesson should take around 30 minutes to complete.

We aim to keep you engaged, teach you something new, and help you feel supported. You can access this course on a computer, tablet, or smart phone. Diabetes Education is very beneficial for you!

Lisa Graham

Lisa Graham is originally from Los Angeles, where she received her Bachelor of Science in Nursing from the University of Southern California. She moved to Atlanta 22 years ago and is now happy to call it home. She is the owner of Renew and Live, LLC; which focuses on bringing diabetes education to the community via group classes. She is the founder of Diabetes You Can Win Foundation, Inc. which serves those in underserved communities.

Lisa realized early in her career that she enjoyed working with people with diabetes. Through diabetes education, she realized she could make an impact on the lives of her patients, and that is what led her to become a Certified Diabetes Educator. Diabetes is near and dear to her heart; Lisa’s husband has Type 1 diabetes, and her daughter has Type 2 diabetes, so managing diabetes is what she does on and off the job.

Lisa believes that health education is essential to addressing the rising incidents of diabetes, obesity, and heart disease in our communities. She is committed to serving her community and dedicated to making a difference in the lives of people with diabetes.

This is an eight-lesson, self-paced course for people who have been diagnosed with Type 2 diabetes, or want to know more to help a family member or friend who has been diagnosed. In this course you will learn about:

  1. The signs and symptoms of diabetes and the 5 numbers that matter most- A1C, LDL cholesterol, A1C, kidney function, and eye exam results.
  2. How to test your blood glucose and why testing is important to good diabetes management.
  3. How to develop a healthy eating plan that helps control your blood glucose and decreases your risk of complications.
  4. Exercise and why getting more active is important to your diabetes management and overall health.
  5. The different medications used to treat diabetes and why it is important to take the medication prescribed by your doctor.
  6. Avoiding complications and staying healthy with diabetes.
  7. Dealing with the stress of diabetes.
  8. Some of the challenges people with diabetes face and how to stay on track with your diabetes plan.
